they real or NOT ??? A common question that runs out when you compare with pictures of a celebrity with perfect breasts. The question is then recounted when this celebrity comes six months later for a movie premiere with their perfect now fuller breasts.

It's hard to tell these days - with fashion ribbon, push-up braces and Photoshop - that has real breasts and has implants. Although implants are the case, with breast augmentation up to a science, most enlargements can appear and feel like natural as the actual breasts. And with more women who choose conservative, more natural protective systems, it's even harder to tell.

Courtney Cox was rumored to have had augmentation after splitting from her ex-husband David Arquette. Her breasts certainly look like they have added volume while the rest of her body looks the same. "It's hard to tell," says Beverly Hills, CA, plastic surgeon Leslie H. Stevens, MD. "It could be the bathing suit that's giving the illusion of a fuller chest," says the doctor.

For natural-looking breast augmentation, that's what you want: "hard to tell." Nowadays, surgeons can "really tailor the surgery to patients so that it is going to look natural. We have so many implant shapes and sizes, we can pick ones that conforms to the patients  proportions perfectly," he says.

Jaime Pressly "looks like she is better endowed now," says Dr. Stevens. "Before she had a boney sternal area and now she doesn't."

For those that might want to chalk the new fullness up to motherhood, Dr. Stevens says that added volume like this is unlikely from pregnancy. "Most women don’t gain volume after pregnancy, they suffer atrophy."

Reality star Audrina Patridge has denied rumors of breast augmentation, saying that her changing shape is due to a bone condition in her chest, but it looks pretty obvious that she has had a major increase in breast volume over the past couple of years. "Definitely looks like she has implants," Dr. Stevens.

"On a breast augmentation you want to consider proportion and size. Women that are petite and thin aren’t going to have big, giant chests naturally."

At the age of 42, Catherine Zeta-Jones has perfectly round, uplifted breasts which look very natural.

Whether they are fake or not, is tough to determine. The breasts certainly look soft and not hard, the way a bad augmentation would look. The goal with implants is to have them look soft like Catherine's. "Your own breast tissue bounces when you run and walk, it compresses softly when you give someone a hug. Your breasts after augmentation should still do that. And if they are soft, they certainly will look much more natural. Making an adequate pocket during the surgery and then, a committed program of breast implant massage to keep the pocket edges from sealing back together is key," says Newport Beach, CA, plastic surgeon Douglas Hendricks, MD.

Nicole Richie has fluctuated in weight a lot though her time in the limelight, however, her breasts seem to have had the greatest influx in volume. "They look good, and they don’t look unnatural," says Dr. Stevens, who believe the star did indeed undergo breast augmentation.

If you are considering augmentation and want soft, full-looking breasts, Dr. Hendricks suggests "patients avoid tight wrappings and bandages after surgery, so that from day 1, the implant has freedom to move. That, coupled with a few simple exercises, can go a long way to ensuring a natural look and feel."

Singer Christina Aguilera doesn’t only have a big voice, she has a rather large bosom. The star, who is rumored to have undergone a transaxillary endoscopic breast augmentation (where the implants are placed through an incision in the armpit to avoid scars on the breast itself) has never commented publicly on her enlarged chest, despite having scars present under her arms.

Scars are of course a dead giveaway of an augmentation but often can't be avoided. "The primary option for enlarging the breasts if one doesn’t want a scar on the breast is to go through the armpit. However, no one can guarantee how a scar is going to look and the phrase “invisible scar” is an unrealistic concept. Almost all scars on the body are forever visible. They may not be obvious but you can expect at minimum a pink line for a few months that will change to a white line forever," says New York plastic surgeon David Rappaport, MD.

The Jackson family is known for their cosmetic procedures, and that includes Janet Jackson, who looks to have had her breasts enlarged. "There is a huge difference in size," says Dr. Stevens when looking the her past pictures and her bust size now.

LeAnn Rimes, whose body has been under press scrutiny since her teen stardom, looks to have undergone augmentation. "If she has implants, its not a good job," says Dr. Stevens. "The breast is falling," he continues.

Why does this happen? The singer, who has recently lost a good amount of weight,  doesn’t naturally look to have a lot of soft tissue covering the implant, which might be the reason it looks unnatural. Also, "she may have the implant on top of the muscle," says Dr. Stevens, which works for some patients and not for others

Kelly Rowland "definitely has implants," says Dr. Stevens. The singer has confirmed her surgery for and reportedly "loves" her new breasts thanks to their natural look.

The best way to get a good result is to find a surgeon who is well trained in breast surgery. Finding a board-certified plastic surgeon who has extensive experience with augmentation will most often give the best opportunity to get a great result," says La Jolla, CA, plastic surgeon John D. Smoot, MD.
